Yes, Use Attribute 2058 should be "Uncontrolled Term".

Yes, Tag 22 should be "Uncontrolled Term".

In parallel to the subelement "Uncontrolled Term", "Controlled Term" should
be assigned Use Attribute 2074.

The subelement "Uncontrolled Term" repeats within the element Subject Terms
Controlled. I think your interpretation will produce the required behavior
insofar as Subject Terms Uncontrolled is viewed as a grouping of the
component subelements. I don't know what additional clarifying language
would cover all possible data models, though.
